This form needs to be completed by all students in order to participate in the swim classes and to attend field trips.
ALL STUDENTS must have a completed and updated EMF on file to attend.
All students should complete this form, even if forms have been registered at another school.
Please read and complete each section in order for your child’s enrollment forms to be completed.
This form must be consistently updated and accurate, as it will inform us of the names of the adults that may pick up/or meet your child at the bus stop.
This form will register your child using the date it is received by the Educational Coordinator’s office.
