Service & Support Administrator (SSA):
A Service & Support Administrator, or “SSA” as often called, is the main point of contact for most adults who are determined eligible for services. Everyone who is enrolled on a Medicaid Home and Community-Based Services Waiver is assigned a SSA. Sometimes a SSA may be assigned to other people, including children, who have extensive planning needs but do not receive waiver-funded supports.
A Support Administrator uses a person-centered approach that focuses on what matters most to you. You are at the center of everything that happens. The role of the Service and Support Administrator is to:
- Talk with you and your family.
- Identify your needs and concerns and develop a plan (Individual Service Plan or ISP) to address these.
- Focus on your strengths, skills, and abilities.
- Find solutions, preferably ones that are community-based.
- Make sure the services and supports work best for you.
- Help you develop advocacy skills.

About Eligibility:
Trumbull DD provides support and services to people with developmental disabilities of all ages. We plan services based on the needs and goals of each person. We call this person-centered planning. Support and services can be provided in the home, school, at work, and other places in the community and help you to live a meaningful life.
The chart below outlines eligibility criteria by age and what steps to take to begin the eligibility process. Once you reach out to us, an Early Intervention Specialist or Intake SSA will contact you to guide you through each step of the process. We look forward to assisting you.
IMPORTANT: Eligibility must be redetermined at age 3, age 6, and age 16 for continued services.

Redetermination for Trumbull DD services is required at ages 3, 6, 16, and any time there is a significant change in a person’s needs or abilities. Redetermination is a process that assesses if you are still eligible for services, and, if eligible, which supports you may receive.
